Choledochal cyst is associated with an overall cancer risk of 10% to 15%. 45 Fig. 22 Choledochal cyst. The cystically dilated bile duct is lined by reactive columnar cells, and here it is seen to be associated with a mild chronic inflammatory infiltrate and periductal fibrosis. Choledochal cyst was classified into five types by Todani in 1977 according to location, morphology, and number of dilatation: type I (saccular or fusiform dilatation of extrahepatic bile duct), type II (diverticulum of extrahepatic bile duct), type III (dilatation of duodenal portion of common bile duct; choledochocele), type IV (multiple dilatations of extrahepatic bile duct with or without involvement of intrahepatic bile ducts), and type V (Caroli disease).

Several ballooned hepatocytes are present, and one of these contains a Mallory-Denk body. One should note that Mallory-Denk bodies often, but not always, are found in ballooned hepatocytes; however, not all ballooned hepatocytes contain such inclusions. Ballooning degeneration is considered a hallmark feature of alcoholic and nonalcoholic steatohepatitis, but it also occurs in acute viral hepatitis, chronic hepatitis C with concurrent steatohepatitic features, autoimmune hepatitis, neonatal giant cell hepatitis, druginduced liver injury, and ischaemia/reperfusion injury in liver allograft.

Fig. 23 Choledochal cyst. This cystically dilated bile duct is lined by columnar cells. Focal denudation and underlying congestion are noted. Choledochal cyst is a cystically dilated bile duct lined by biliarytype epithelial cells with varying degrees of erosion, ulceration, and regenerative and metaplastic change. Periductal fibrosis and inflammation of varying degrees also may be found. Choledochal cysts can contain a smooth muscle layer allowing distinction from the inflamed, dilated common bile duct observed in obstruction.
